Immunization Information
Immunizations need to be completed BEFORE your child begins the 2012 - 2013 school year. Be sure to schedule early with your child's provider to make sure the immunizations are completed before Sept 1. The DHS chapter 144 of the Wisconsin Administrative Code requires that all students have the necessary vaccinations or a waiver form on file within 30 days of admission (the 1st day of school). Please make sure that your child has all the required immunizations for his/her incoming grade level. When your child does receive an immunization please bring written documentation from the healthcare provider to your child's school nurse.
***PLEASE NOTE THAT STUDENTS ENTERING 6TH GRADE ARE REQUIRED TO HAVE A TDAP IMMUNIZATION. This is a once in a lifetime immunization containing tetanus-diptheria-and acellular pertussis.
If you choose not to have your child immunized for religious, personal, or health reasons. You must fill out and sign the bottom portion of the Student Immunization Form. On this form, you must indicate the reason and specifically what immunizations you are waiving. This must be done each year.
Healthcare providers may fax immunization documentation to 608-486-4587 for MS/HS or 608-486-4057 for Elementary.
