Medication At School
The Bangor School District recognizes that your child may need over the counter medication, such as cough drops, Tylenol, or Ibuprofen during the school day. The School District will no longer be supplying over the counter medications.
The State of Wisconsin in now requiring that all over the counter medications be supplied by parents/ guardians. If you want your student to have OTC medication, (i.e. Tylenol, Ibuprofen,cough drops) it must come in the manufacturer original container with the label and directions clearly visible. It must also be labeled with your child's name.
A medication administration form must be filled out for each OTC medicine. A parent/guardian must provide a signed a Medication Administration Form BEFORE any medication can be administered. **This form can be printed from the link at the bottom of this page or is available in the office of each school. A Healthcare provider signature is not required. Unless the dose to be given exceeds the manufacturer recommended therapeutic dose, then a provider signature is required.
When your child needs prescription medication to be given during school hours, a healthcare provider must give written instructions and sign a Medication Administration Form. These are available at provider’s offices. A parent/guardian signature is also required on the Medication Administration Form. The provider can fax the signed form to 608-486-4045 for the elementary and 608-486-4587 for the middle/high school.
